Clear explanations, informed decisions and respect for privacy are central to a constructive care experience. Ask the team for the hospital’s current patient-rights and privacy information and how to raise a concern.
Ask about patient support
Understand your care
Ask clinicians to explain your assessment, proposed treatment, alternatives and relevant risks in language you understand. Tell the team if you need interpretation or another communication format. You should have the opportunity to ask questions and discuss your preferences before decisions about your care are made.
Manage information sharing
Ask how your information is used and how to request access to records. Tell the team whom you authorise to receive updates or assist with arrangements. If a relative or representative is acting for you, confirm the consent or authorisation process that applies to the information being shared.
Raise questions or concerns
If something is unclear or does not meet your needs, ask for the appropriate staff contact or patient-support process. Explain what happened, when it occurred and the outcome you are seeking. Ask how the concern will be acknowledged and what further information may be needed.
