Write down the diagnosis or treatment recommendation you would like reviewed and the decisions you are considering. Explain whether you are seeking clarification, a comparison of options or advice about further assessment. A focused question helps the reviewing clinician identify which records and investigations are relevant.
Prepare the supporting information
Gather recent specialist letters, investigation reports, treatment summaries and a medicine list. Contact the coordinator to confirm the submission method and whether imaging files, pathology material or translations are required. The team can clarify the scope and any fees before you decide to proceed.
Discuss the outcome
Ask whether the review will be provided in writing, through a consultation or both. A clinician may recommend further information or an examination before reaching a conclusion. Share the opinion with your current treating team so that any decisions can take account of your full circumstances.
