Follow-up appointments help review progress, results and the next stage of care. Arrange the timing with the treating team and check what information should be ready for the review.

Agree the purpose and timing
Ask whether the follow-up is intended to assess recovery, discuss results or review a treatment plan. The appropriate timing depends on your clinical circumstances. Confirm whether you should see the same clinician and whether the appointment should be in person or can be considered remotely.
Prepare an update
Keep a brief record of changes since your previous visit, including symptoms, recovery milestones and questions about medicines. Complete any investigations requested for the review and confirm how the clinician will access the reports. Bring the previous care plan if you have a copy.
Keep the care team informed
Tell the team if a test is delayed or you cannot attend at the recommended time. Ask how this affects the follow-up plan and whether another arrangement is needed. After the review, clarify who will manage ongoing care and when the next contact should take place.
