Describe the main concern, when it began, the investigations already completed and the treatments tried. Include a current medicine list and the questions you want answered. Contact the coordinator to agree which documents are needed and the appropriate method for submitting them.
Understand the review scope
Ask who will review the information, what the review is intended to decide and whether there are fees. Depending on the available records, the clinician may suggest further information, an in-person consultation or additional investigations before advising on treatment options.
Use the findings to plan
Request a clear explanation of the proposed next step and any information that remains uncertain. Confirm appointments and practical arrangements separately before travelling. If your condition or circumstances change after the review, tell the team so the plan can be reconsidered where necessary.
