Relevant medical records help a clinician understand your history and the question you would like reviewed. Contact the international coordinator first to agree an appropriate way to submit them.
Ask how to submit records
Start with a brief enquiry
Provide a short description of the reason for review, your preferred language and a contact method. Ask which records are needed and how they should be submitted. Do not place medical reports, identification documents or other personal health information in a public discussion or comment area.
Organise the relevant records
Prepare a concise medical summary, recent reports, a current medicine list and details of previous treatment relevant to your enquiry. Keep documents in date order and retain the originals. For imaging, ask whether reports are sufficient or whether the original image files are also needed.
Confirm the submission requirements
Before sending documents, confirm accepted formats, any translation requirements and whether consent is needed when someone submits on your behalf. Ask the team to acknowledge receipt and explain the next review step. Submitting records does not establish eligibility for a treatment or confirm an appointment.
