Continuity of care matters after an international visit. Before leaving, agree how your treatment summary, results and follow-up needs will be shared with you and your clinician at home.

Leave with your care summary
Ask for a summary of the assessment and treatment, relevant investigation reports, a current medicine list and any recovery instructions. Confirm which documents are ready and how outstanding results will be delivered. Keep copies available for your local clinician and retain the original records.
Agree responsibilities
Clarify who will manage medicines, wound care, investigations or rehabilitation after you return. Arrange any recommended local appointments before departure where practical. If a remote hospital review is proposed, confirm whether it is available from your location and what information is needed beforehand.
Know how to follow up
Ask when the next review should take place and whom to contact for routine questions. Discuss any specific changes that should prompt local medical assessment. Your follow-up plan should reflect your condition and treatment, including how your local clinician can seek further information with your consent.
