Discuss a professional referral
Frame the clinical question
Include the reason for referral, the established diagnosis if known, the current clinical status and the question you would like the receiving team to address. Identify any time-sensitive issue so that urgency can be discussed appropriately.
Agree the records required
A useful referral may include a clinical summary, medication and allergy lists, previous treatment and relevant imaging or pathology. Obtain the necessary patient authorisation and agree the appropriate transfer method before sending identifiable records.
Plan communication and continuity
Provide the referring clinician’s contact details and clarify how findings and recommendations should be returned. Discuss which team will arrange further tests, communicate with the patient and provide ongoing care after the specialist review.
