Stereotactic Surgery for Brain Injury

Medical History

Mr. Stanley Padley, a 57-year-old male, sustained a brain injury following an operation in 2009. Magnetic Resonance Imaging (MRI) revealed softening lesions in both hemispheres of his brain, accompanied by cerebral atrophy. The injury left him unable to speak or eat due to the loss of sensation in his tongue. A percutaneous endoscopic gastrostomy (PEG) tube was inserted for feeding purposes.

Stanley experienced difficulties walking and relied on an electronic wheelchair for daily activities. He was admitted to Beijing Puhua International Hospital in April 2015 due to speech and mobility disorders that had persisted for over six years.

Medical Condition Prior to Stereotactic Surgery

Before the intervention, Stanley's condition was as follows:

  • Speech: Unable to protrude or move his tongue; unable to speak
  • Mobility: Difficulties moving due to increased muscle tension in lower limbs, particularly the right lower limb. Right elbow joint was in a slight extended-contracture position.
  • Muscle Strength:
    • Left upper limb: 5-/5
    • Right upper limb (proximal): 4/5
    • Left lower limb: 4/5
    • Right lower limb: 4-/5

Stereotactic Surgery for Sequela of Brain Injury

Upon admission, Stanley underwent a comprehensive treatment program that included:

  • Neural function improvement therapy to enhance recovery of brain functions
  • Promotion of cerebral blood circulation and metabolism to facilitate healing and plasticity
  • Traditional Chinese Medicine (TCM) to complement Western medical treatments
  • Rehabilitation therapy to regain motor skills, balance, and coordination

Medical Condition after Stereotactic Surgery

Upon completion of the treatment program, Stanley's general condition improved significantly:

  • Swallowing Function: Improved; able to consume banana slurry and other semi-liquid diets
  • Tongue Sensation: Reported having some feeling in his tongue
  • Mobility: Increased flexibility in limbs
  • Muscle Strength:
    • Left upper limb: 5-/5
    • Right upper limb (proximal): 5-/5

Follow-up

  • Able to walk much better than before without any supports
