Medical History:

Mrs. Hu, a 58-year-old woman from China, was admitted to BPIH on April 29, 2007, after experiencing sudden unconsciousness while bathing. She had been unresponsive for two days prior to arrival.

Pre-Operative Condition:

  • Sudden onset of unconsciousness
  • No verbal response; unable to communicate
  • Urinary and fecal incontinence
  • Two episodes of non-projectile vomiting containing gastric contents only
  • Slight to no movement of the right limbs, with improved left limb mobility after 30 minutes
  • Cerebral CT scan: Subarachnoid hemorrhage (SAH) and high-density shadows (free blood) in brain areas
  • Routine Blood Test: WBC 9.4×10^9/L, NE 81.6%, RBC 3.5×10^12/L, Hgb 114g/L, HCT 34%, PLT 136×10^9/L
  • Diagnosis: Ruptured cerebral aneurysm

Surgical Repair:
Under general anesthesia, the surgical team performed:

  • Clipping of the giant posterior communicating artery aneurysm via a left frontal-temporal approach
  • Resection of the aneurysmal sac
  • Total operation time: six hours

Post-Operative Condition:

  • Stable vital signs with no severe postoperative complications
  • Gradual recovery of consciousness
  • No further episodes of vomiting or loss of consciousness
  • Discharge status: conscious, oriented, and able to communicate normally with family and others

Follow-Up:
Post-operative cerebral CT scans revealed no abnormal signs.
